1. Cheat to win.
Example: Pulling the tights during a pin, hitting your opponent with a concealed weapon, distracting the referee.
Who does that? The kind of heel who only cares about winning. Hey, it's not personal, I just really want the money and fame that goes with winning.
Famous example: Ric Flair, Bobby the Brain, Chris Jericho, Triple H

2. You know sportsmanship? Forget about it. Fight dirty.
Example: Inflicting pain on your opponent just for the fun of it, digging into bloody wounds, brawling and just being vicious.
Who does that? Either a face who is getting revenge on someone who has wronged him or a psychopath who enjoys the suffering of others. It most likely isn't personal and if it is it's because of some perceived but imagined slight their opponent made against them.
Famous examples: Triple H, Cactus Jack, Raven, Big Show, Dudleyz

3. Fight clean then gloat about it when you win.
Example: "Yeah, X-Pac really did a good job in the ring tonight, I was impressed how he handled my consistent and superior offense. There was actually a moment there when I thought he might not lose that bad."
Who does that? Assholes who aren't good in social situations. Yeah, they're egotistical and cocky but, they honestly think everyone aside from them is piss-poor at life.
Famous Examples: Kurt Angle, Chris Jericho, Triple H (again), Mr. Perfect, Jack Swagger

4. Be rude, crude and disrespectful.
Example: Slap your opponent in the face, insult their manhood, their family and their intellect, suggest that you may or may not have had sexual relations with their mother, possibly sister as well. Make everything personal.
Who does that? The kind of person who gets off bullying others. They love making other people feel miserable and are attracted to train-wreck personalities because of this. They go out of their way to embarrass, mock and piss off their opponents and generally aren't pleasant to be around.
Famous Examples: Triple H (again), HBK, Kevin Nash, Scott Hall, X-Pac, Hollywood Hogan, Ric Flair, Justin Credible

5. Be angry for no good reason
Example: Slapping your opponent just because they're in a match with you. Berate them. Yell at them. Shove them. Yell at the referee. Be extremely surprised that that no-good assclown almost got a three-count on you. The world is involved in a conspiracy against you, fuck everybody!
Who does that? Paranoid people with low self-esteem. People who think they deserve more in life but others are singling them out for punishment (and for no good reason!).
Famous Examples: Chris Jericho, Kurt Angle, Lance Storm

*This list is not exhaustive. The famous examples chosen are those who stand out in my mind. There are a multitude of possible wrestlers who could fit under various categories. Also, a surefire way to get heat is to talk shit about the local sports team. Either that or purposely say the wrong city.

Thank you for proving that Triple H is, in fact, the single greatest heel in the history of professional wrestling.

I've got to admit his heel run from fall 99-spring 01 was damn amazing. Definitely the top heel from that Attitude era. Of course it didn't hurt that at the time he was going up against the two biggest faces since Hogan, Rock and Austin. His feud and retirement matches with Foley were just flat out great too.

Undertaker managed to have the best counter for the whole "What" chant on Smackdown during his American Bad-ass phase.

“Say What!? if you sleep with your own sister” followed by a few people saying "What" and then awkward silence for a few seconds (or laughing at the people who said "What").

Not giving the fans what they want always works too, mainly on the indy scene though.

IE, high fliers who refuse to do their high flying moves, or Roderick Strong, when he'd set up for a loud chop, then just not do it.

I'm also a big fan of someone going for a plancha, only to jump over onto the apron and kick the guy.
