I know a heap at two, but I've never heard of anyone reaching three. Has anyone reached three and been fired?

Randy Orton did in 2014.

I believe Eugene was the only one, but it was because he couldn't get a doctor's excuse for one of his meds, which WWE considered illegal.

He's handicapped though he shouldn't be held responsible.

Jeff Hardy?

I think he only had two.

I think he only had two.

"According to sources, Jeff hardy was fired from the WWE on 22nd of April, 2003 because of his drug use, erratic behavior and his refusal to go to rehab"

That was his 3rd strike

This was way before the mandatory three strikes rule was in effect, which was put in place after Eddie and Chris died, so officially, he had two.

Theres a bunch of folk who left on two rather than go to rehab, Eugene & at least one of the Hardys, like Perry Saturn said. They'd all have recieved a 3rd strike if they'd stayed or not been released.
