Not quite always, I'd be willing to bet if they were actually smart enough to run with it, there could be some kind of Double-KO or other draw/no contest that makes both look like they'd given 110%, and nobody has to take a loss and look weak or lose momentum.

How about Hunter versus Rollins?

1. Hunter defeats Rollins for the title at a PPV event.
2. Rollins wins back the title next night on RAW.
3. Hunter once again defeats Rollins for the title at a PPV event.
4. Hunter successfully defends the title against Rollins on RAW.
5. Sheamus cashes in and becomes the champion.
6. Rollins defeats Sheamus, Cena, and Hunter at a PPV event to become the champion.

Market the Wrestlemania championship match as a triple threat bout where Rollins defends the belt against Triple H and Cena. If Triple H or Cena win, they equal Ric Flair's record on the grandest stage of them all.

Next night on RAW, Cena cuts a promo on Ric Flair and demands him to return his World Heavyweight Championship. Ric Flair comes out with the big gold belt and tells Cena that he will have to earn it back. Hence, Cena and Flair (out of retirement) square off in the middle of the ring. Cena defeats the 16-time champion to rightfully own the World Heavyweight Championship. The Authority come out and announce that since the World Heavyweight Championship was retired, the victory will only be symbolic and will not count as Cena's 16th title reign. Talk about screwy finishes :)
