Was it WCW that had this rule at one time? I can't remember.
Were they the only ones to have this rule?

Most feds had it back in the day. WCW were the last company to enforce it I think. They stopped it around 91/92.

Read Jesse Ventura's book. He lost his first match by DQ after he threw the guy over the top.
A lot of companies had it - SMW used it, and WCW kept it until the late nineties I believe.

Didn't Bill Watts introduce that? Or did he just ban any top tope activity in general?

Watts banned all top rope moves. Wrestlers got DQed if they did them.

I remember that, I beleive even in the WCW video games early on that it was banned for jumping off the top rope.

That's why WWF were clever, they made it so that if someone throws a guy over the top rope they win. That's why WCW couldn't invent the RR.

Well, it was something that looked like it could injure somebody, and, in the NWA and WCW, it could make for a good heel tactic for an intentional DQ to keep a title or to do behind the ref's back for heel heat. Then again, someone going over the top rope and landing on the concrete NEVER got injured, right?
Maybe I should ask John Cena...
