I actually watched it just to see what the hell Code Black was, great marketing by ABC
But yeah Code Black refers to a bomb inside a patient. Some guy got shot with a bazooka rocket and it didn't explode.

There was a live, undetonated explosive in a patient's chest, being held stable by a paramedic. If the paramedic let go, it'd blow up. In the end, the paramedic got flustered and let go, but another doctor reached in and grabbed the explosive. The end.
Edit: Crap, Innovator beat me to it.

I've never watched this show before until last night, and I was really impressed. Dr. Grey annoys me, but the episode was well-written and well-acted. Plus :y: to the Christina Ricci and Kyle Chandler (I think is his name. The guy from Early Edition and King Kong) cameos.
Although I totally saw the thing about the pregnant doctor's husband coming.

Yeah I was tempted to watch just to see what Code Black is but decided not to. Wierd, I would of thought that it would have been like an outbreak or something. A bomb of all things, hmm.
