How many companies are there?
How much money do the fighters make?
Do most companies fight in a ring? UFC has the octagon, what do other companies have?
A few companies. Biggest are Pride and UFC. K-1 do some MMA but mainly a kickboxing group. WFA is restarting and they seem to putting a lot of money into this. Strike Force are doing well in San Jose. Cage Rage is doing well in England too. Other companies with a good name but smaller are King of Cage and Hook 'n' Shoot (very good indy).
Money depend where you work and where on the card. UFC for example - Bottom guys in dark matches make as little as $2000 for winning and $2000 bonus. Some of the top guys will make over $500k easy from fights alone. Personal sponsers rake in cash. Tito Ortiz, Chuck Liddell, Matt Hughes and probably Rich Franklin and Forrest Griffin will clear $1 million this year.
In the U.S, the MMA law is everything in a cage. Safer and makes for a better visual. Some shows still use rings. Pride uses the ring in Japan but will have to use a cage if they ever promote in the States unless they get permission to use a ring (not unlikely) but they have certain rules in fights they will definately not be allowed to use such as longer rounds and kicks and knees to downed opponents.

This comes from wikipedia, which I know isn't the most reputable source, but I"m too lazy to search for anything else.
Although "There are no rules!" was the tagline, the term was not strictly true, the UFC did operate with a limited set of rules. There was no biting, no eye gouging, and techniques such as hair pulling, headbutts (http://en.wikipedia.org/wiki/Headbutt) and groin strikes (http://en.wikipedia.org/wiki/Groin_attack) were frowned upon, but allowed.
