Here's what I currently have:

ADVENT 3215
Pentium 4
2.66GHz
512MB DDR
120GB
PSU 400W
GeForce 7600 GS OC

I'm planning to buy a new computer, but I really have no idea where to start.

Would I be best buying parts invidually, or just buying one ready made?

Also, this computer will mostly be used for games, so I'd like to know what are the must have parts to stay with the demands of modern games.

I'm not thinking about budget right now, as I can sort that out later once I know what I need.

Building your own is the ONLY way to go for gaming. You can buy gaming rigs, but they cost tons more than if you were you buy each part individually and assemble.

For the motherboard, you want something that supports LGA775 processors (aka dual core, quad core, etc.) and MAYBE SLI/Crossfire (i.e. lets you use two video cards at once)

You can also do what I did and look at a gaming rig you want and find out what parts are being used in them and build a list like that.
