The first ever was Bad News Allen (Brown) Vs Bret Hart for Stampede wrestling

The first WWF one was HBK Vs Bret

The first famous one was HBK/Razor Mania 10

According to Wikipedia (and elsewhere), Dan Kroffat vs Tor Kamata was the first ever ladder match in 1973, 9 years before Hart/Allen.

I prefer my story, as it contains an answer that makes me seem correct.

I always thought the Hart/Allen match was first, was it a belt hanging from the ceiling, climb up the ladder and grab is match?

Or just a 'use a ladder as a weapon' match? (The Dan Kroffat vs Tor Kamata one)

Not sure, but it was for $2000 apparently.

yea, that one was the same deal as Bret/Bad News. Money hanging from a bag above the ladder. I think most of Stampede's ladders were like that

Could've sworn I read somewhere last week, that the first ever Ladder Match in North America was in Canada.

I believe Bruce Hart actually got the idea from England, where, and this is not sarcasm, techno music was playing during the match and the ladder led to a disco ball that had to be taken down.

And given the history of pro wrestling, not entirely unbelievable.

But effing hilarious either way. :D
