<font color=goldenrod>Yeah it's been done I'm sure...but whatever. Title should be pretty self explanatory.
Anywho, I'll break mind down into the two sports I follow:
<b>Best Wins - MLB:</b> Game 6 of the 1986 WS has to be right up there even though I wasn't alive to see it...in terms of what I did see though, Robin Ventura's Grand Slam single in Game 5 of the 1999 NLCS, Todd Pratt's walkoff HR in Game 4 of the NLDS, and Benny Agbayani's 13th inning walkoff in Game 3 of the NLDS all stand out for me. Good times.
<b>Worst Losses - MLB:</b> Kenny Rogers' meltdown in Game 6 of the 1999 NLCS was horrific. Beltran taking strike 3 looking in Game 7 of the 2006 NLCS also gutted me, I would have felt so much better if he at least went down swinging. And, of course, pretty much every game last September...easily the worst sports moment ever for me.
<b>Best Wins - NFL:</b> The Giants have given me some memorable wins but obviously, none of them can top Super Bowl 42...easily the best sports moment of all time for me. I'm not sure that could ever be topped for me outside of the Mets winning the World Series on a walkoff HR or something.
<b>Worst Losses - NFL:</b> The tank job the Giants pulled in the 2002 NFC Wild Card round against the 49ers really got to me, the game just collapsed on them. Super Bowl 35 also sucked because they just plain didn't show up to that game.

Win: NFC 2006 Divisional Playoff Saints over the Eagles
Loss: NFC Championship Bears over Saints
The hook and ladder play against the Jags a few years ago could be mentioned as the worse loss.. They ran a miracle kick return where like every skilled player touched the ball (Michael Lewis, Stallworth, Joe Horn, Deuce, Jerome Pathon, etc) to come within onepoint with no time left and then John "Small Hands Smell Like Cabbage" Carney shanked the PAT.. That game would've gotten us into the playoffs that year..

best win - SB over Rams
Worst Loss - the SB loss the the Bears... although the 1996 loss at Foxboro to the Broncos sticks out because of Shannon Sharpe's whole "hello mr president"...
the SB loss to the Giants and the AFC championship loss to the Colts also both stick out...
MLB
best - 2004 ALCS game 4
worst - game 6 vs Mets. or the ALCS the 2003 where Pedro gave up the home run.

best - soo many bests... I'll go with game 6 this year just cause I was there.
worst - 1985 game 6... dropping the title to the Lakers on our home court.

:( 131-92 loss to the Celts game 6. Worst loss ever.
Sweetest win : Lakers game 4 against the Spurs, Fisher's .4 second shot.

Best: hmmm, let me check, yep, Aaron Boone's walk off Game 7 homer to beat the Sox in the Playoffs. Legendary
Worst: tough one. I'm going to say Game 7 of the 2001 World Series when Luis Gonzalez hit that bloop single to win the series. You've probably seen it as they seem to show it 20 times a year.
Another tough one though was the Jets loss to the Steelers a couple of years ago in the playoffs when they had the game won about five times, but our fucking kicker forgot how to do the only job he was hired to do.

Best: Game 7 '03 ALCS. The Aaron Boone home run is absolutely legendary. Honestly ripped off my pants in a fit of joy after it. Was like rubbing my head on the carpet and shit, amazing
Worst: In retrospect the whole '04 ALCS, 4 straight losees ugh. Especially Game 4. But Game 7 of the '01 World Sereis makes me sick everytime.

Best: '02 playoffs vs. the Colts. Most epic season ever, with 'You Play to win the game' and Chad actaully playing like an NFL QB. Fucking sneaking into the playoffs in week 17 (I was there) because the Pats beat the Dolphins in overtime, and then they fucking crushed the Packers like 41-0 to get in. Then first round of the playoffs 41-0 again againsty the Colts. Amazing
Worst: '04 playoffs. Doug Brien. 17 seconds left. 38 yard field goal. hits the post. Overtime, for the win 42 yards, shanked it. Completely gutted. Amazing game besides that too

Worst loss ever
Eagles/Bucs 2004 NFC title game. The Bucs never won a game in like 50 degress or less in basically their entire history, and that day was like the coldest day. It was also the last football ever played in the Vet, and the Bucs had a horrible record there.
Within almost the first 10 mins the Eagles were up 10-0. Everyone was happy, but the Eagles couldn't find the endzone the rest of the game. Down 20-10 Dovann McNabb leads the Eagles down the field in an herioc drive. Everyone optimistc that the Eagles will score, and possible recover an onside kick. Of course that can't happen, because #5 has to throw to his favorite target Ronde Barber, and Ronde Barber runs it back 90 yards for a TD. Then me subsequently falling on the floor wanting to cry. Final score 27-10 Tampa Bay.
Now the Bucs went on to play the Raiders which was a team on the brink of destruction, and of course Tampa ending up winning the Super Bowl.
I don't think I can recover from that.

Worst: Wings beating the Pens in Game 6 this year :'( Colts/KC Playoff game a few years ago where Dante "X-Factor" Hall had like 300(2) Punt/Kick Return TDs.
Best: Next year when the Pens win the Stanley Cup!

Leafs: Best, Borchevsky's goal to knock out detroit in the first round of the 94 playoffs
Worst: Kerry Fraser's no call of Gretzky that led to the Leafs beig knocked out
Pats: Beating the Rams for the Superbowl
Worst: Obviously last year
Blue Jays: Joe Carter's walk off homer to win the world series
Worst: Losing the pennant on the last day to the Tigers
Jazz: MY fav is Stockton's last second three pointer to beat Houston in 98
Worst: Jordan's jumper over Russel to finish us off that year.

Best: Game 4 5 6 and 7 of the 2004 ALCS, Patriots Superbowl 36
Worst: 2003 ALCS and this years Superbowl. In both cases I literally sat there and couldn't believe it

Best win : Beating the Patriots in the AFC Championship game in 2006. Winning the Superbowl was just a formality. This felt so wonderful
Worst loss : I was gunna say either the playoff game in Miami or the game against the Steelers where Vanderjerk shanked game winning field goals wide right on purpouse cause he was most likely paid to. I was also gunna say the 41-0 chin checking we took against the Jets in the Playoffs. But i think i'm gunna go with the 1994 AFC Championship game when Harbaugh's hail mairy fell right into Aaron Baileys lap as he laid there in the enzone and all he had to do was put a finger on it and it would have been the greatest play in NFL history. Instead it rolled off his stomach and onto the ground. So many gut wrenching memorys about that game. Kordell Stewart getting that touchdown even though he ran out the back of the enzone right in front of the ref. Quentin Coryatt dropping a game sealing int that was right on his numbers. Lamont Warren being shoe string tackled behind the line when all he needed was like 2 yards for the 1st down to seal the game and there was a hole the size of my apartment to run through. The Colts should not have even been there. They werent suppose to beat big bad KC in Arrowhead but they did. And back then, going to Arrowhead really did mean something. Such a tough way to end a season being so close to going to the superbowl.
