Sid poopy-pants certainly qualifies.

Here's one that no one will mention...

I want to say it was Summerslam 97, maybe? Mankind v. Triple H in a cage match. Mankind ripped open his shirt, and there was supposed to be a Dude Love heart painted on his chest, but it had sweat off during the match. It just looked like he ripped his shirt open for no reason at all.

Shockmaster, obviously and alot of the Sid/ Luger promo fuck ups. Another moment was when Goldberg "supposedly" knocked himself out when he hit the pole at Starrcade 99 (which led to Hart slipping while putting in the figure four on the ring post, and that led to one of the like 15 concussions Bret suffered in that NWO 2000 period)
