What happened to Sydneys mom?

Ive seen all the episodes now by renting the first 3 seasons and she doesnt die in any of the episodes. But during the new season they said something along the lines that she was dead and that Jack killed her because he thought she put a hit out on Sydney.

Can someone tell me what happened to her and possibly what episode cause this is really bothering me?

At the very end of S3 when Sydney is looking over those documents with like ultraviolet light or whatever and she nearly breaks down and then Jack comes in and says "you weren't supposed to see that" or whatever, that was the documentation for Jack basically executing Irena.

Keep watching the new stuff though, it should be cleared up definitively pretty soon.
