How did Hell in a Cell start?
 
I picked up the Best of Hell in a Cell DVD's, which are awesome by the way. Was there any sort of storyline reason to introduce the HIAC or just a new match? I gathered from the first match that Commissioner Slaughter had something to do with it.

From what I remember, The Undertaker and HBK's previous match at Ground Zero had a lot of outside interference and a bunch of ref bumps, so their next match was supposed to be a cage match, but instead WWE introduced the Hell in a Cell in order to keep everyone out, HBK in and the refs safe.

Like what Juan said, there were too many interferences in their matches, so there was only one logical way to solve the issue.....Hell in a Cell.....

I thought the most logical way would have been to be men, put their differences behind them and shake hands. That would have set a good example for the kids.

They needed a new fresh match to get buyrates up in a competitive ppv atmosphere that they were losing in.
WarGames had always been a hit, so they put one of those over their ring and stuck their top talent in it.
