It was created during development of the game. R* cut it out, presumably by by just making the programming code into comments (in other words, it's still in the programming code, but it doesn't run, so unless you can see the code, you wouldn't know it's there). Some guy finds out how to undo the edit and shows everyone how to do it. Jack Thompson and some other people flip out. R* takes back a shitload of copies and puts new ones out with the scene actually removed, not just edited, and the rating changes from M to AO, making it the first non-Japanese game to get that rating.
