Custom belts can cost over $1000. Not many promotions are willing to place so much money on a fake championship, which is a common mistake. I've known many feds that have used the WCW Heavyweight Championship replicas because it lacked a logo and was big.
ECW was a promotion on the rise and took recognition in the fact that they needed real title belts. Some other small promotions take that step too, but usually can only afford small belts.
