Depending on the specs you could probably set it up with something like MythTV (under linux). It's not too demanding, especially if you use you use your desktop as the backend. It supports virtually everything and you can get remotes and what-not for it.

Though no matter what, you'll likely be hard-pressed to push out HD content on something that old without upgrades (and maybe not at all).

Personally, to stream video to my TV I use a modded XBox with XBMC.
