You hotkey the button, not the spell. I liked to keep my instants or most important things right by the wasd keys for quick access. I would rebind a lot of lesser used commands in order to get the most out of my layout. The settings should be somewhere in the menu no matter if you have an add-on or not.

Like, pretty much used the whole left side of my keyboard from this line - 6, t, g, v and over to the left. Anything past that was too far a reach and not useful for being quick. Use a combo of shift+ and alt or ctrl+ and you have all the hotkeys you need. Buffs and the like can be assigned to the keys you're not going to use for quicker access. 7 and over pretty much, or I could just click buffs as they weren't needed as often.
