Most of the focal points of the show are pretty JD-centric. JD's relationship arc with Cox, JD and Turk, JD and his relationship arc with Elliot, JD and the janitor. Obviously that is because he was the narrator, but still. If you bring in somebody new at this point, you only have a few residual things left that were mostly just side stories. I think Scrubs could do fine without Carla, or anyone else, but JD would be like Jerry Seinfeld not wanting to be in Seinfeld and them having them call the show Costanza.
