He was still him though. He had ties to the world. That's why he cared. The further his ties got from him, the less he cared.
As his the people originally in his life died, or left him, he drifted further away. When Spectre left him, he had nothing and he left Earth as he put it.
If it wasn't for these ties proving to be strong, and drawing him back in, he never would have returned.
The only thing that upset him was the cancer thing, which again, had to do with people in his life prior to becoming "inhuman"
