The best thing for Matt right now may be to get a release. He's not going anywhere anymore. He hit his peak, and it was wasted, sure, but he's gone nowhere but down, with slight blips on the radar here and there. He needs a new start. If he does indeed get released, the first thing he needs to do is take some time off and lose some weight. Let him get into ring shape, and see where it goes from there.
