Based on the ads, my understand is that they take two "ugly ducklings" (who usually fine but don't fit the correct mold) then make them go through plastic surgery and dieting for a while with no mirrors.

Then the person finally sees themself in a mirror and crys a bunch. Then some beauty experts judge which of the two is more beautiful.

According to the commercials this is supposed to culminate in the ultimate beauty competiton to crown "The Swan" (which I don't see the point of as the women have already gotten their plastic surgery, so some bogus beauty contest really means nothing)
