Dishonorable Mentions: Million-Dollar Mania, Mae Young gives birth to a hand, The JR "Colon Surgery" skit
Was pretty much going with this...except I'd also mention the Austin beer truck to the Rock/McMahon as a mention for best moment and having the Mae Young "birth" as the worst.
