The best one I've thought of so far is Sandow cashing in when both Del Rio and Cena are exhausted, probably after Cena hits the Attitude Adjustment, and due to the rule that the MitB can be cashed in at anytime, the match becomes a triple threat match and Sandow immediately pins Del Rio and becomes champion.
I'm just assuming that Cena wins, an upset Del Rio destroys his arm, Sandow cashes in, then Cena leaves to finish recovering.
