He just comes across as a bit weird at times and that he has a carefree attitude towards his health and safety. That's about it really.
When he gets called a lunatic and the announce team talk about how crazy he is, it just feels forced and not true. If he actually did crazy things or do them more often, then it wouldn't be so bad. If he jumped off the Hell In A Cell for example, that's crazy, or in an Elimination Chamber match he put himself through the chamber glass for some reason, be it to get to an opponent or to inflict more damage to an opponent, that's crazy. Even setting a table on fire and going through it with his opponent would help live up to him apparently being a lunatic.
The craziest thing I've see him do is throw himself on drawing pins/thumb tacks when he hit Jericho with Dirty Deeds at Extreme Rules.
The problem is, he doesn't get many opportunities to do crazy things because the majority of his matches are "traditional" matches, where DQ can happen, so that means no weapons, no structures, no leaving the ring for more than ten seconds.
If the Hardcore Division was still alive, he would be the perfect fit for it in this day and age. His hardcore matches would be memorable ones and very entertaining and he would easily live up to being a lunatic. Instead, he's expected to be a "lunatic" without using weapons or structures to his advantage and be within close range of the ring. I can't think of how you can be a lunatic and win matches by playing by the rules every week.

I don't think he needs to use more weapons to be a lunatic. I think he does well, I don't think he's a lunatic though. He's more of a common man. He does crazy things every once in a while but the WWE couldn't book a lunatic for nothing. He named a potted plant Mitch, is that lunaticish? I liked when he was stealing belts and running around WWE HQ. Just needs more legit crazy things to do. I hated the Ambrose Asylum and thought it was out of character for him to be hosting a talk show.
