Face/heel dynamic is designed by how crowds respond to you. If they're paying money to come and cheer you on, then you are a babyface, even if you drown puppies in front of them. If they are coming to boo you and pay to see you fall, then it doesn't matter if you cure all their diseases -- you are a heel. Austin wasn't "technically a heel" because he beat up old men and didn't respect anyone. He was the biggest babyface of all-time because people loved him.
