Got a response from THQ regarding the glitches, here is what they said:

Hello Nick and thank you for contacting us - yes, it does seem as if your game disc is not being scanned correctly. Usually for quickness it's best to return your game disc to the shop and I think your disc may well be damaged but I should suggest perhaps trying the game in a different PS2 just to kill off the possibility that your PS2 is not having problems scanning a disc that's not damaged. Very remote possibility I agree, particularly if your other games are playing fine but it's just to separate out if it is the game disc that is not working rather than the PS2 itself. Hoping this helps. Our sincerest apologies for the inconvenience this is causing you.
