I dont know if anyone noticed (or if this counts for that matter but) I think the ending to the ultimate x match at 2005 bound for glory was scewed up because of 2 reasons.

1) Sabin's, Bentley's and Williams' reactions were one of being really really confused after Pettie won the match by just catching the "X" when it fell to the ring instead of climbing the cable.

2) They had a rematch on the following impact tapings.

I remember reading that the X fell off twice in the match. The first time they stopped the match, re-attached it and restarted the match.

The second time it dropped into the hands of the guy that was supposed to win so they just rang the bell
