Like always, the rules in wrestling can be changed to fit the storyline. The Undertaker was knocked unconscious when Edge cashed in and the match started with Taker laid out, ditto with CM Punk cashing in on Edge.

But in recent cash ins, the champion has had to be up and standing before the match begins. They need to be aware that a match is going on.

I think that Mark Henry's ankle being messed up superceded him being knocked out though in this instance.

When they show up and see that the mini trons say "(PPV) is two nights away!" they get confused and try to order it for Thursday, find that Impact is the only wrestling on that day and stop watching entirely because of how ridiculous TNA is.

I was concerned in case Survivor Series was a one-off, however, it would appear that Cody Rhodes is keeping the knee pads. The man is now officially ready for the main event.

There's a difference between Henry cutting himself shaving and spilling kool-aid all over and Big Show mangling his leg with a chair at Survivor Series, leading medical personnel to not clear him to compete. Cena wasn't "medically prohibited" from wrestling. That's the difference. Bryan not getting to cash it in made perfect sense. Henry wasn't cleared to wrestle, therefore he couldn't have a match.
