I personally believe that "Hell in a Cell" matches are meant to end feuds. To have a random "Hell in a Cell" match for the title would be stupid.

They can have the WWE Championship match open the show as a singles match and end the night with the World Heavyweight title being defended in the "Hell in a Cell" structure.

Maybe Nash will return around Survivor Series as a mystery "5th man." If Punk goes over, then Big Sexy will likely have to jump off Punk's shoulders and hope Punk moves his knee pad close enough to Nash's head to look somewhat like a GTS. Oh well, I guess he would need to pack on considerable fat to reach that milestone; I'm okay with a leaner Punk, so long as he can hoist Nash for the GTS if/when they resume their feud! I suppose that genetically, different guys have different muscle carrying capacities per inch; with or without steroids, Scott Steiner, Ultimate Warrior, and Johnny "Rellik" Stamboli were genetic freaks to pack so much lean mass on their frames!

I'm sure Cena will push through and work Hell in a Cell, but assuming that he is absolutely out, and if Randy Orton cannot work the PPV due to filming commitments, I think they'll go with Triple H. It'd be pretty easy to set-up (as far as impromtu things go), and it would be a "big match" for PPV. You could bring up Paul Heyman's lawsuits against the WWE, or have him and Punk try to hold up the WWE for more benefits their way, leading to The Game coming out and announcing that he would step into the Devil's Playpen -- a match he is infamous for -- to rip the WWE Title from Punk.

If Cena doesn't wrestle at "Hell in a Cell", the championship match better be a non cell match.
