I think it's all scripted to an extent. The rookies probably wing their promos on the fly but still know their topic ahead of time. With the other challenges I'm sure they know who is supposed to win so everybody else has to intentionally lose/do crappy (or if you're Titus O'Neil, stumble over).

Fan vote likely means nothing, but it makes for good storylines. If they actually DID count fan votes, it is probably only reflected in the mid-range guys with WWE knowing who they want to have in certain spots
