It looks like he's doing something with his hand on his head as he gets thrown into the cage and then the ref does definitely get down and possibly take something from Punk's hand.
What makes it more suspicious is that Punk quickly thrust his head up in a weird way as if he's trying to keep the blood from dripping down from the top of his head and become visible yet. Then after the fist drop he keeps his head down and lets it drip down making it seem like the fist is what busted him open.
