A lot. The ideas were mostly his. But I'm sure there were also lots of other ideas, and further details that were rightfully refused.
That's because Vince and Co. were there to filter his ideas. He was no doubt a creative powerhouse, and came up with a lot of great stuff. But having a leash seemed to be important.
I think Russo and McMahon worked together to create the genius of the attitude era. I don't think it's right to give either full credit, or strip due credit away from either.
