At what PPV or maybe Raw did Goldberg pull an Ultimo Dragon and slip on the ramp? I think it was his match against Jericho, but I'm not sure.

It was at Summerslam last year as he was comming out and he does that punch-kick combo thing he fell over as he tried to do the kick. It was pretty funny. If you noticed after that he was always a lot more careful when he done the kick.
