You missed one... (Maybe more, but this is the one I know) Bret Hart vs. Shawn Michaels back in 91-92 on a Coliseum Home Video... It was the first WWE ladder match... The ladder match goes way back to the 70s I've heard, though... Just not in the WWE...

Also, I seem to always remember an early Hardcore title match on RAW in 98 I believe... Big Boss Man vs. Mankind in a ladder match...

NO, Ladder matches originated in the 80's in Stampede Wrestling. I think, but don't quote me on this, the first ever ladder match was Bret vs Dynamite Kid.

But Stampede wrestling took the idea from Mexico

Also, theses are some Ladder Matches, I remember seeing in WCW, I know I'll proberly miss a good bit of them.

US Title - Syxx v. Eddie Guerrero
3 Team Ladder Match - Jung Dragons v. 3 Count v. Kurageous/Noble
US Title - Jeff Jarrett v. Chris Benoit
US Title - Jeff Jarrett v. Chris Benoit (Happened the next night after the other one)
Konnan/Mysterio v. Boogie Knights ( I think I saw this on Nitro in the later WCW Days)

wasn't there a mankind/rock ladder match on raw in the run up to wrestlemania 15. Big Show interfered and choke slammed foley from the top of the ladder, leading to their winner referees main event match at WM15
