What's an extremely scary film?

Can't go wrong with The Shining. Though if you're looking for a good date/horror flick, I'd go with An American Werewolf in London.

A few other recommendations:

Event Horizon
Jacob's Ladder
Alien (the first one, not the action-y sequels)
The Omen (the original, not the crappy remake)
The Serpent and the Rainbow
Salem's Lot
